Burning in groin and buttock sounds like nerve pain; but is it?


Hi there,
I have been having severe pain for more than two years across my lower back and into my left hip and buttocks.

I wake up every morning with extreme burning and numbness in the buttocks and upper hamstring. I have had a lumbar mri as well as xrays of lumbar and sacrum.

I have seen so many doctors without a diagnosis. Just recently the pain has gone into my left groin. A lot of my pain is in the joint in the buttocks and hip. At times the burning gets really bad in the buttocks around the tailbone area. Any information would be greatly appreciated. Thanks.

Hello Colin,
These two little tests will rule out sciatica; let's just start with them, and then take it to the next step. Do them after no analgesic or anti inflammatory medication for 24 hours.

1. Bend slowly forwards, and tell me exactly what you feel. Now repeat by going backwards and to the side. Do it gently as it can worsen the condition.

2. Sitting in a normal kitchen chair, ask the missus to raise your foot so the leg is parallel to the ground, and then slowly flex your head; not your whole body.

Do it first with the normal leg and then with the naughty limb. Tell me exactly what you feel.

3. Perhaps one more since you have groin pain; lie on your back and pull your knee to your chest, then to the opposite shoulder, and then make a circle using your knee as a lever.

Let me know what you feel. Is the pain in both buttocks?

Burning back of thighs and burning in both cheeks

Hello, i have been suffering for a few weeks with burning in the back of thighs and buttock cheeks. Akso have low back pain. MRI scan came back as normal. Had xray of pelvis and hip and have mild osteoarthritis. I am 45 and had a bad car accident 2 years ago with just soft tissue damage. I have suffered with a dull ache in my right leg for 18 months and thought it was restless leg but i am not convinced anymore. The burning in the back of thigh snd buttocks is a new symptom. Some nights i cannot sleep whichever position i try snd sleep. I cannot cope with this anymore.

There may be more than two conditions overlapping here.

Firstly, if you lie on your back and pull your knee to the chest, then to the opposite shoulder and the drop it into the lotus position, is it much stiffer and painful than the other hip?

Sitting in a kitchen chair, flex your head onto your chest, and then have a friend straighten each leg parallel to the ground. Is it particularly tight in the post leg and lower back?

Standing, if you bend slowly forwards, then backwards and finally to each side does it reproduce the pain in your back or leg?

Finally, it's not impossible that much of this has to do with sacroiliac joint and the piriformis muscle; that's more difficult for you to assess.

Maigne's syndrome should also be considered, and an inguinal hernia can produce many strange symptoms. There are many other possibilities, but I've covered the most common that chiropractors encounter.

A bad car accident always leaves its mark; it's unlikely there was only soft tissue damage.

hip labrum

I have had severe pain in left hip, groin and buttocks for quite some time and it ended up being a torn hip labrum. It can cause all different types of symptoms and pain across lower back, groin, buttocks and leg. I was getting MRIs of my spine and it ended up being the hip.

The torn labrum also cause severe pudenal neuralgia symptoms. A torn hip labrum is something to get checked out. Lots of doctors don't think of it!

With groin pain I always start with looking for either an inguinal hernia or a hip condition; they present very clearly if you examine properly.

It's true that lower back, and more likely a sacroiliac condition can mimic these symptoms.

You probably have an underlying CAM or Pincer deformity, or even a dysplasia. Any family history of hip problems?

Take it seriously, exercise it gently every day, or it will become arthritic.

Debilitating Leg and Abdomen Spasms

I need suggestions. I have been two 2 neurosurgeons and 2 general surgeons. Here is what I know:
*I've never had back pain or trouble in my life. (I'm 35 year old, active female)
* All of a sudden I've started getting debilitating spasms that are in my right leg, from my Toe to my abdomen, everytime I stand, rollover in bed, cross my legs, etc.
*They are started with a stabbing pain in the left side of my groin and heat up my left leg into the left side of my butt.
*They last about 45 seconds-1 minute.
*There was no injury. It literally started as I was standing in the kitchen one day and my right leg gave out. It went into a tight muscle spasm followed by intense, uncontrollable shaking all over me.
*I'm not on medicines. I'm not an alcoholic. Nonsmoker. Never been into drugs.
*The heat/stabbing pain stays on my left while the spasms stay on the right.

I just know I can't live like this and am trying to find suggestions to put me on the right path to getting help. I've already had an MRI and a CT scan. I'm willing to share those results but want whoever is reading this to start with a clean slate and possibly give me some new ideas since no one can seem to get things lined out. It has literally taken me from a very active on-the-go person to someone who is having a very hard time functioning. Please, please help!

If it's in the abdomen then a condition there must surely be the starting point; the femoral nerve runs through through and over the psoas muscle and can certainly be affected in the gut or groin, causing pain running down the front of the leg, and inner lower leg to the foot.

It take it your abdomen has been examined. No findings? No inguinal hernia?

Then an examination of the hip would be my next consideration; lying on your back, pull your knee to the chest, to the opposite shoulder, and then make a circle; any pain or stiffness in the groin or side of the hip.

Drop you hip into the lotus position; what happens?

Let me know. Are there x-rays of your pelvis showing the hips?

I had burning feeling in my right groin that comes and goes; I went to my gynecologist she said it is common in girls and gave me some medications; now the pain is not there and it burns a little less.

Now I am experiencing as if my left buttock is burning; the pain also comes to the sides.

Can you please tell me if it's serious or what?

No, I'm afraid I can't. The rule is that any pain that doesn't go away after a period should be explored further.

It could be a condition like hip dysplasia, something to do with the sacroiliac joint, Maigne's syndrome, or a medical condition that would affect the lymph glands in the groin; an a heap of other possibilities.

Go back to your doctor and tell her what's happening. See if you get her to make a diagnosis rather than just giving you painkillers.

Dear Dr. B,

I was wondering if you could help me. I had microdiscectomy 6 months ago on the right side on l4-5 level. All the pain in the right side is now gone, however, lately I have been experiencing stiffness, sharp to dull pain in the SI-joint, buttock and groin level on the left side that is slightly felt down the left leg too. Right now as I am writing this it doesn't hurt, just feels very tender and weak.

Bending downward slowly doesn't hurt. The slump test and the straight leg test don't hurt at all (granted I did them on my own, with no help from anyone). Yesterday I felt sharp stabbing pain in that area while walking that lasted for about several seconds. It's been achy since then.

Please help me determine what this might be. My spine is in a very bad condition. I have (had at the time of my last MRI) a slight annular tear and a small bulge on the l3-4 level, and a degenerated disc at l5-s1.

All your help is appreciated.

Thank you Ana for taking the time to write a proper little report; so many folk write one or two sentences in not so smart phone grammar, and wonder why I get grumpy!

Your lower back clearly needs care, mostly on your part. If you were consulting me, I'd make three suggestions.
1. Absolutely every morning for the rest of your life do some simple lower back exercises every morning before getting out of bed.

2. Accept that there are some things you shouldn't do. Obvious ones like not moving the deep freeze, but also choosing your chairs carefully, and bending and twisting as in vacuuming and sweeping leaves in the garden may have to end. Don't fight it; certain activities are just beyond your back and if you continue with them you'll end up yet more surgery.

3. A short course of chiropractic adjustments and then a monthly visit.

One more; start keeping your chicken bones, freeze them, and when you have a pile make a bouillon for your soups. It contains the cartilage that will help your own.

If the SLR and Slump are negative (I can see you've been reading at the site!) then this may be coming from your hip or sacroiliac joint. If you pull your knee to your chest, and make a circle, does it hurt in the groin?

Hello Colin,
Let me preface what I have to say, that trying to advise and make a tentative diagnosis from x-rays and our preceding discussions is at best a hazardous business. The only reason I attempt it is that you've done the rounds without resolution.

There's a two way relationship between the hip and the midlumbar spine, via the femoral nerve. It works in both directions, and is often something of the chicken and egg.

You have small pincer deformities in both hips, especially the left, which explain the stiffness you have in the hip. It's called femoro acetabular impingement syndrome.

Then there's straightening of the lumbar lordosis, and in fact a slight kyphosis at the thoraco-lumbar junction but not much else that I could see abnormal in your lower spine. I couldn't open the report, by the way.

That suggests that there may be a facet syndrome in the upper lumbar spine, but that's a long shot. I would examine that area carefully that's all.

My gut feeling, to sum up, is that this is a hip condition, mild and fortunately with no degenerative changes, probably affecting the sacroiliac joint and possibly the mid to upper lumbar spine.

I would be looking to the orthopaedic tests of the SIJ and the femoral nerve stretch test.

These are the kind of findings that I treat on a regular basis, but each and every one is different, has its unique features and puzzles.

Perhaps print all this out, and look for a good chiropractor in your area. Sorry I'm not closer!

the other thing that has been happening over the last month is the top of the hamstring just under the butt cheek is extremely painful on both sides especially after sitting for a period of time.

heating pad

A question I have is that I pretty much live with a heating pad, either sitting on one at work to relieve pain or sleep with one on my hip or lower back, is this alright to do for this type of condition. I have been told many different things about this.

I would say until a diagnosis is established, use whatever helps. No side effects from a heating pad, but you might find alternating heat and cold better still.

Hey Barry. I have a copy of a lumbar mri. Would that be useful for you.

They are difficult to send, and my thinking is that this is probably not a lower back issue. Something in the pelvis. SI or hip.

the strangest part is that several specialists have done the straight leg test with me and I can do it with no problems at all. Both sides do not cause more pain for me. My pain is a deep aching burning type of pain.

Then it's not sciatica which is good news; that's why bending forwards doesn't hurt in your leg.

When bending forward the pain is in the very lower part of the sacrum underneath the buttocks cheak and the hip. When I sit for a long time I get burning in the tailbone area; it's really bad.

This is all pointing to a sacroiliac and possibly hip condition. It's good that it doesn't go down the leg when you bend.

I also two different neurologists that both said that I have lack of reflexes in my right leg.

If in the right leg then I'm less concerned.

Once again, if you bend slowly forwards, is the back of the left leg very tight? Much tighter than the right?

Again, with the sitting sciatic stretch (called the Slump test; use the search function at chiropractic help), is it tight in the left hip and groin, or at the back of the left thigh.

Have you done Lasegue's test.

I need all this info to get a clear picture. And a copy of the radiograph.

I had a surgeon do the Patrick's Fabere test. When this test was done i found that the left side was much tighter and painful compared to the right side. I had pelvic xrays done which supposedly showed nothing wrong. I can send you copies by email? Right now when I wake up in the morning I have extreme pain in the left glute area as well as the left hip and across the lower back. Also both of my legs and feet go numb the longer that I sit during the day. I will send my xray results soon.

If it's digital, you need a few computer skills. If a plain radiograph, tape it up on a window with blue sky behind and take a photo with digi.

Obviously, not being able to examine you it's difficult, but my thinking is a hip problem that is causing the SIJ pain; it can be vice versa.

That tight Fabere with stiffness and pain (in the groin? side of the hip? sacroiliac joint?) is very significant. Something is locking up, and I'm thinking probably the hip. The negative xray report is interesting; I'm wondering if you have a pincer or CaM deformity. I need to see the pics.

We're getting there!

When I raise my left leg to the chest and do the circle test I feel pain in my hip joint and extreme tightness. The right side is normal. Just recently the pain has moved to the right buttocks as well.

This strongly suggests that at least in part this is a hip condition.

Using that same search this site function at chiropractic help, type in Patrick's Fabere test.

What's the difference between right and left, and where precisely is the limitation?

It's time for a pelvic xray with a spot of the left hip. Can you send copies to contact for me to view? Plus a copy of the radiologist's report.

Did any of the specialists find any sensory deficit in your leg, loss of reflex or specific weak muscle paresis?

Lastly, sitting, with your groins exposed, put a little oil on your thumb and run it down from the ilac crest, through the groin and down the inner thigh. Compare right and left. Is there a difference?

When I bend forward or backwards I don't feel much pain just tightness in the back. But what do you feel in the legs?

When I do the knee to chest I have no pain at all. That's good.

When I do a leg raise test in a chair the right side feels normal and the left side is more tight feeling. I have tried all kinds of nerve pain medication and none have helped. Could it be bone pain? This suggests the sciatic nerve is probably involved.

Now ask the boss to raise your right straight leg whilst lying flat on your back. It's called Lasegue's test. When the leg gets tight, ask her to dorsiflex the toes and ankle. Repeat with the naughty leg. What's the difference?

Very important; when you bend forwards is the back of the left leg much tighter?

Use the search function to find the straight leg raise of Lasegue.

Be as precise as you can in your replies if you want a useful answer.
